Brand Levitra left US shelves as a business decision. Generic vardenafil film coats stayed: 2.5, 5, 10, and 20 mg. Staxyn ODT is a different fill. Reviews that praise 'the 20 mg' without a QT med list are incomplete reviews.

02Fifteen percent bioavailability and a 60-minute median peak

Absolute bioavailability is about 15 percent. After a single 20 mg dose in healthy men, peak usually arrives between 30 minutes and 2 hours, median 60 minutes, fasted. Terminal half-life of parent and M1 is about 4-5 hours. Clearance is 56 L/h. Vss is 208 L. Protein binding is about 95 percent for parent and M1.

M1, the desethyl metabolite, sits at about 26 percent of parent levels and has about 28 percent of vardenafil's in-vitro PDE5 potency, so it accounts for about 7 percent of total activity. CYP3A4 does most of the work; CYP3A5 and CYP2C help. Feces carry about 91-95 percent of an oral dose as metabolites; urine 2-6 percent. A mean 0.00018 percent of a 20 mg dose showed up in semen at 1.5 hours.

Those numbers explain a fast commercial story and a thin oral fraction. They do not excuse a fatty plate. They do not beat tadalafil's 17.5-hour tail on a Sunday. The onset pulse is the stopwatch homework.

Dose proportionality held across the labelled range. That means 20 mg is roughly double the exposure of 10 mg in a typical adult, not a magic faster onset. Median Tmax stays in the same hour-wide band. Men who swallow 20 mg because 'Levitra is the fast one' still wait for arousal. Pediatric use is not indicated. Carcinogenicity studies in rats and mice at high multiples of the 20 mg human dose did not show a vardenafil tumor signal. That leftover from the PI is not a reason to start at the ceiling.

04Brand Levitra left US shelves; the INN did not

US brand Levitra was discontinued for commercial reasons, not a new safety withdrawal. Generic film coats remain the product a US window can actually price. Import blister art that still says Levitra may be old stock or a parallel pack; read the INN and the milligrams. A 10 mg leftover from a trip is not a 20 mg ceiling script.

Drugs.com and WebMD review threads often describe men splitting 20 mg because the first sample 'felt too strong'. That matches the label's 10 mg start better than it matches shop merchandising. Headache, flushing, nasal congestion, and dyspepsia were common (≥2 percent) and showed a dose trend across 5, 10, and 20 mg.

Need a long untimed window instead of a ceiling tablet? Tadalafil 10 mg. Need first rungs without a QT footnote this loud? Sildenafil 25-50 mg. The PDE5 align sheet is the three-way map.

Review sites still host men who dissolve a chip under the tongue. That is not a labelled film-coat route. Staxyn is the ODT product, 10 mg, on the tongue, no liquid. Home-split 20 mg under the tongue is improvisation. Flushing, stuffiness, and a racing pulse in those reviews often faded when the milligrams fell. That is the label talking through anecdotes: start at 10 mg, or 5 mg if age or liver says so, and treat 20 mg as a later step.

05Staxyn ODT is a different fill than a 20 mg film coat

Staxyn is vardenafil 10 mg orally disintegrating, placed on the tongue, no liquid. It is not interchangeable with a 10 mg film-coated tablet because systemic exposure is higher. It is certainly not a 20 mg. Taking both on the same day is a duplicate INN, not a clever onset trick.

High-fat meals cut film-coat Cmax by 18-50 percent. On Staxyn, a high-fat meal did not move AUC or Tmax and cut Cmax by 35 percent; trials ran without regard to meals. Both products may be taken with or without food. Neither product is a nitrate loophole.

Alcohol at 0.5 g/kg with vardenafil 20 mg did not potentiate hypotension over a 4-hour observation in healthy men, and plasma levels of each were unchanged. That is a pharmacology study, not a license for five drinks on a first 20 mg. Orthostasis still shows up in real rooms.

Storage below the labelled room-temperature band matters in a Barcelona summer car. A warped film coat is a ruined assay, not a stronger tablet. Keep the blister dry. Imprint codes change by maker (the generic PI example uses L 50 through L 53 on round coats). Confirm 20 mg on the foil every refill so a 10 mg orange tablet does not get swallowed as the ceiling. One vardenafil product per day. ODT plus film coat is two products.

06Ritonavir 49-fold and the 2.5 mg caps that retire 20 mg

Ritonavir 600 mg b.i.d. with vardenafil 5 mg: AUC ×49, Cmax ×13, half-life stretched to 26 hours. Cap: 2.5 mg once in 72 hours. Indinavir 800 mg t.i.d. with 10 mg: AUC ×16, Cmax ×7, half-life ×2; cap 2.5 mg in 24 hours. Saquinavir, atazanavir, ketoconazole 400 mg, itraconazole 400 mg, clarithromycin: same 2.5 mg / 24 h cap.

Ketoconazole 200 mg, itraconazole 200 mg, erythromycin: 5 mg in 24 hours. Cimetidine 400 mg b.i.d. did not move 20 mg bioavailability in volunteers. Grapefruit can raise levels modestly; the practical counsel is to avoid it rather than 'adjust the 20 mg'.

Organic nitrates remain a class contraindication. Alpha-blockers need a stable background and a low start. Priapism over four hours, sudden vision loss, sudden hearing loss: stop and get care. Those rows outrank any coupon on the 20 mg ten-count.

Cobicistat sits with the potent CYP3A4 list on later generic labels. Boosted HIV regimens change names; the 2.5 mg cap logic does not. A 20 mg ten-count mailed to someone on a booster is the wrong box even if the coupon print is $44.21. Bring the full antiviral row, not a brand nickname, to the prescriber. Qivana will not sort that list by email.

Caps that make a 20 mg foil the wrong box

• Ritonavir: 2.5 mg / 72 h
• Several other potent CYP3A4 inhibitors: 2.5 mg / 24 h
• Erythromycin or ketoconazole 200 mg: 5 mg / 24 h
• Nitrates / riociguat: contraindicated

07Food drops Cmax 18 to 50 percent on a fatty plate

Two food-effect studies on the film coat found high-fat meals reduced Cmax by 18-50 percent. The Canadian monograph also notes a high-fat meal may delay Tmax by an hour. The US label still allows dosing with or without food. The commercial 'fast peak' story assumes a fasted swallow.

If the first 20 mg 'did nothing' after a heavy lunch, you have not finished the assay. Retry 10 mg fasted before you declare the ceiling useless or hop INN. Diabetic neuropathy and low desire will still blunt any PDE5. Speed marketing does not repair a nerve.

Grapefruit and Seville-orange products can raise exposure. Skip them around a vardenafil night rather than titrating the 20 mg against juice. Keep one product, one day.

Orange juice was not expected to clash in the interaction notes that flag grapefruit. Seville marmalade is the bitter exception some leaflets mention. If a first 20 mg felt 'too strong' with stuffiness and a mild pulse rise, the next move is a lower film coat, not a second INN the same week. Log three 10 mg nights with a described meal before anyone reprints the ceiling ten-count. Travel across time zones follows the local clock, not the leftover jet-lag story. One film coat per calendar day where you swallow it.

08Why 10 mg is still the start even when shops push 20

Recommended starting dose for most patients: 10 mg orally as needed, about an hour ahead. Increase to 20 mg or decrease to 5 mg based on effect and side effects. One tablet a day. That is the entire as-needed ladder in one paragraph, and shop title tags skip the first sentence.

Consider 5 mg as the start at age 65 and over (elderly men showed 34 percent higher Cmax and 52 percent higher AUC). Child-Pugh B: start 5 mg, do not exceed 10 mg. Child-Pugh C: do not use vardenafil tablets. Mild hepatic impairment raised Cmax and AUC only 22 and 17 percent after 10 mg; moderate raised them 130 and 160 percent.

Renal impairment down to CrCl under 30 mL/min raised AUC about 20 to 30 percent; dialysis populations were not studied. Alpha-blocker combinations start at 5 mg (2.5 mg with certain CYP3A4 inhibitors) after the alpha-blocker is stable. None of those rows appear on a 'Levitra 20 mg Price' tile.

Forced-titration work with terazosin 10 mg and tamsulosin 0.4 mg showed outlier standing systolic drops (under 85 mmHg or a fall over 30 mmHg) in a large share of volunteers when Tmax was stacked. The terazosin-plus-vardenafil simultaneous-Tmax arm ended early. Symptoms of hypotension pulled several subjects out. That is the clinical reason a first 20 mg on top of an alpha-blocker is a poor idea even if the GoodRx tile is cheap. Stabilize the alpha-blocker, start low, stand up slowly.

09QT, class IA, and class III before anyone prices 20 mg

Vardenafil may prolong the QT interval. Avoid it in congenital long-QT syndrome and in people taking class IA or class III antiarrhythmics. That warning is louder on this INN than on the sildenafil or tadalafil cards, which is why the QT glow pulse exists.

The thorough QT work used 10 mg and 80 mg (four times the highest labelled dose) against placebo and moxifloxacin 400 mg in 59 healthy men aged 45-60. QTc shifts at one hour (near peak) sat in the same range as the moxifloxacin control, depending on the correction method. Mean heart rate rose 5 beats/min at 10 mg and 6 at 80 mg. A later combination study showed additive QT effect with another QT-active drug.

Eighty milligrams was picked because it covers concentrations seen when 5 mg meets ritonavir 600 mg twice daily. That is also why the ritonavir cap is 2.5 mg every 72 hours, not a 'small 20 mg'. Rhythm drugs hide on psychiatric and antibiotic lists too. Ask for 'heart-rhythm medicines', not only amiodarone by name.

Moxifloxacin was the active QT control, not a suggested combo. Adding a macrolide for a chest infection in the same week as a 20 mg date-night plan is a CYP3A4 and QT double hit. Defer the tablet or cap the dose with the prescriber. Sotalol, dofetilide, procainamide, and quinidine belong on the same avoid list as amiodarone. If the med list is messy, pick sildenafil or tadalafil only after a clinician says the class is still open. Do not self-switch to dodge a QT note.

10Seal on the 20 mg ceiling
