Intravascular volume depletion and other antihypertensive drugs can affect the safety of the combination. A dry, over-diuresed chart is a poor night to debut tadalafil.

In those patients who are stable on alpha-blocker therapy, Cialis should be initiated at the lowest recommended dose. For as-needed ED that lowest labelled tablet is 5 mg, not this vault's usual 10 mg debut.
If an optimized Cialis dose is already in place, a new alpha-blocker starts at its lowest dose. Stepwise increases can drop pressure further.
05Can a second PDE5 cover a weak night on the alpha-blocker?
A second PDE5 the same night is never the workaround. Usual as-needed frequency is one tadalafil tablet a day. The alpha-blocker row does not create an exception.

06What does stable on the alpha-blocker mean?
Patients should be stable on alpha-blocker therapy prior to initiating a PDE5 inhibitor. Men who are already dizzy or hypotensive on the alpha-blocker alone are at higher risk when tadalafil is added.
If the alpha-blocker is new this week, wait for that dose to settle before anyone writes 10 mg as needed.
