01What sits between courses on exam
Exclude pregnancy, ovarian enlargement, and cyst formation before the next block. Delay the next course until those are gone. A pelvic exam belongs before every course.

03Why about six cycles is the long-term ceiling
Long-term cyclic therapy is not recommended beyond a total of about six cycles, including three ovulatory cycles. The indication chapter and the dosage chapter both carry that ceiling.
Prolonged use has been linked in epidemiology language to a possible rise in borderline or invasive ovarian tumors. Infertility itself is a risk factor. Duration still matters.
04When 100 mg is the only labelled increase
Rise only if 50 mg did not ovulate. The increase is 100 mg daily for five days, two 50 mg tablets as one swallow, as early as 30 days later. If 50 mg already ovulated, raising the milligram has no labelled advantage.

05How three non-ovulatory courses force a rethink
If ovulation does not occur after three courses, further clomiphene is not recommended and the patient should be reevaluated. The first course is 50 mg. A later course may be 100 mg. Three blanks still end the run.
Primary pituitary or ovarian failure will not yield to a fourth strip. That is why the workup is repeated instead of the bottle.
06When three ovulatory cycles close the course
The dosage chapter is blunt. Three ovulations without pregnancy end the labelled recommendation until someone reopens the case. Male factor, tubes, and lining still sit on that revisit.
A shop that ships another thirty 50 mg tablets after three ovulatory months is skipping that sentence. The vault names the same stop.
