Pregnancy is ruled out. So are ovarian cysts that are not polycystic ovary syndrome, undiagnosed abnormal uterine bleeding, liver disease or a history of liver dysfunction, uncontrolled thyroid or adrenal problems, and an organic intracranial lesion such as a pituitary tumor.

Estrogen status is checked. Low estrogen is less favorable and still does not automatically bar a trial. Endometrial biopsy is expected before therapy in older patients because endometriosis and endometrial carcinoma rise with age.

06What to take to the pharmacy window

Bring the written line, the intended cycle start, and any prior visual-symptom history. New blur, spots, or flashes stop the tablet. The vision-flag pulse is the stop note.
