01How ACTH and cortisol normally cycle
A fall in free cortisol stimulates ACTH. A rise in free cortisol inhibits it. ACTH is low about 10 pm and peaks about 6 am. Cortisol then peaks between 2 am and 8 am and falls toward midnight.
That diurnal loop is what a nightly or split pharmacologic dose flattens. Cushing's disease loses the loop in the other direction. Long daily divided steroid therapy can copy some of those high-corticoid findings.
02Why recovery time is not one number
Recovery depends on the dose and the duration. The insert will not print a single week when the axis is 'back.' During whatever window you are in, stress can still demand hormone again.
Do not schedule a kitchen stop because an online chart promised 14 days of recovery. Ask the clinician who knows the weeks and the milligrams.
03What daily pharmacologic doses do to ACTH
ACTH production is inhibited. Cortisol production follows it down. The person then depends on the tablet for the glucocorticoid work the adrenal used to do. Stopping the tablet overnight leaves that work undone.
Prednisone is grouped with short-acting corticoids that suppress the axis for about 1 and 1/4 to 1 and 1/2 days after a dose. That window is why a missed swallow after weeks is louder than it looks.
04What stress can demand during recovery
Trauma, surgery, or illness can require the dose to rise for a period consistent with the patient's condition, even while the plan is to come down. After a full stop, hormone therapy may need to be reinstituted if stress arrives before the axis has recovered.
Life-threatening hypotension, severe weakness, nausea, and dizziness are among the adrenal-insufficiency associations. Those skip this probe.
05How a morning single dose compared with q6h
A single morning prednisolone 10 mg suppressed the axis less than a quarter of that dose every 6 hours. That is one reason once-daily morning is the default. It does not mean a months-long 10 mg habit can stop cold.
Alternate-day mornings exist to let HPA activity return on the off day. Those plans are written. See morning dosing for the hour, not for the stop.
06Where the stop and the stair live
Abrupt-stop rules sit on do not stop a long course cold. Whether every course needs a stair sits on when a taper is needed. The stair counselling sits on the taper pulse.
