01What peptic ulcer history already flags
Active or latent peptic ulcer is on the caution block with diverticulitis, fresh anastomoses, and nonspecific ulcerative colitis. Adding a shop NSAID on top of that block is a second gut insult.
Black stools, coffee-ground vomit, or new sharp upper pain skip this probe.
02What concomitant NSAIDs do to the gut
The Teva prednisone insert says aspirin or other NSAIDs plus corticosteroids increase gastrointestinal side-effect risk. That is the pairing sentence. Ibuprofen is in that 'other' group.
Peptic ulcer with possible perforation and hemorrhage already sits on the corticosteroid adverse list even without an NSAID. The stack makes that list easier to hit.
03When to ask before adding a shop NSAID
Ask before you add ibuprofen, naproxen, or high-dose aspirin for 'steroid aches' or a headache on 10 mg. Acetaminophen questions belong to the clinician, not to this page inventing a substitute.
The prednisone vault stays on the 10 mg lock. This probe stays on the NSAID sentence.
04What to watch for on the stack
Stomach pain that is new, vomit that looks like coffee grounds, stool that turns black, or fainting with abdominal pain. Those are care, not a different brand of ibuprofen.
Do not start leftover antibiotics because the abdomen is quiet. Quiet can be the mask. Read the infection pulse if fever also looks small.
05Why aspirin also has a salicylate-clearance note
Corticosteroids may increase the clearance of chronic high-dose aspirin. Salicylate levels can fall while both are on, then rise when the steroid is withdrawn. Aspirin should be used cautiously with corticosteroids in hypoprothrombinemia.
That clearance note is not a reason to add aspirin. It is a reason for the clinician who already wrote both to watch the pair.
06How food or milk does not cancel an NSAID stack
Gastric irritation may drop with food or milk. That help is for ordinary steroid irritation. It is not an antidote to ibuprofen plus prednisone.
Antacids between meals are written for large steroid doses. They are not a license to add an NSAID either. See 10 mg with food.
