01What to tell the immunizer

02When to defer routine shots
Routine administration of vaccines or toxoids should be deferred until corticosteroid therapy is discontinued if possible. 'If possible' is the clinician's call, not a home delay of a needed inactivated shot.
Bring the last-day plan to the immunization desk. A burst that ends next week and a months-long 10 mg habit are different desks.
03Why chickenpox exposure is a separate call
Exposure to chickenpox or measles is a counselling line of its own. Seek advice without delay if exposed. That call is not the same as booking a live vaccine.
The infection pulse holds the fever watch. This probe holds the shot stop.
04Why killed vaccines may still have a weaker response
Killed or inactivated vaccines may be administered. Antibody response to toxoids and to live or inactivated vaccines can still be diminished. The insert says that diminished response cannot be predicted.
Do not assume a shot 'did not count' and repeat it from a pharmacy fridge without the clinician who wrote the 10 mg.
05What replacement-dose exceptions are
Indicated immunization procedures may be undertaken in patients receiving nonimmunosuppressive doses as replacement therapy, for example Addison's disease. That sentence is narrow. A 10 mg anti-inflammatory burst is not replacement therapy unless someone wrote it that way.
Do not borrow the Addison's exception because 10 mg looks small next to a 60 mg morning.
06What the vaccine contraindication says
Live or live-attenuated vaccines are contraindicated in patients receiving immunosuppressive doses of corticosteroids. That is a stop, not a 'ask if you feel well.' The immunizer needs the current milligram and the weeks.
Corticosteroids may also potentiate replication of some organisms contained in live attenuated vaccines. That is the biologic reason under the stop.
