Some generic metronidazole labels discuss LFTs around Cockayne use. The Flagyl tablet insert's line is contraindication. Do not open a 250 mg pack as a monitored trial because a blog said 'just check LFTs'.
Severe hepatic impairment from other causes is a different row: Child-Pugh C gets a 50 percent Flagyl dose cut. Cockayne is not that row.
02What to say at the visit that would write 250 mg
Name Cockayne syndrome before the prescription is printed. A covering dentist may not have the genetic diagnosis on the dental chart. Bring it up yourself.
If the infection still needs an anaerobic drug, that choice belongs to the clinician who knows both the organism and the syndrome. It is not a leftover Flagyl decision.
03Which liver facts are not this stop
Child-Pugh A and B raised AUC24 about 54 and 53 percent after 500 mg IV; those groups are monitored, not auto-halved. Child-Pugh C was +114 percent and is halved. None of that rewrites the Cockayne ban.
Darkened urine in about one patient in 100,000 is a metabolite pigment the insert calls clinically unimportant. It is not Cockayne liver failure.
04How fast the liver reports started
Latency from drug start to signs of liver failure has been as short as two days. That is why 'take the first few and wait' is the wrong plan in this diagnosis.
If Cockayne syndrome is on the chart and someone already swallowed metronidazole, that is a same-day clinical call, not a metallic-taste question.
05What the contraindication says
Flagyl tablets are contraindicated in patients with Cockayne syndrome. The adverse-reaction section repeats the liver-failure reports, including fatal outcomes with very rapid onset after systemic metronidazole.
A 250 mg dental course does not get an exception because the milligram is smaller than 500 mg. The stop is the syndrome, not the stamp.
06What this stop is not
It is not the metallic-taste complaint. It is not the three-day alcohol hold. It is not first-trimester trichomoniasis. One diagnosis, one contraindication.
Family members sometimes share leftover antibiotics. Do not hand a Cockayne relative your 250 mg blister. Flagyl is not OTC and this stop is not optional.
