01What dialysis removes
A hemodialysis session of 4 to 8 hours removed 40 to 65 percent of a 500 mg dose, depending on membrane and session length. If the dose cannot be timed away from dialysis, a supplement after the session may be considered.
A 7.5-hour peritoneal dialysis session removed about 10 percent. CAPD does not need a Flagyl dose rewrite on that row. ESRD can pile up metabolites; watch for metronidazole adverse events.
02What 'lasts' means after the final swallow
The last 250 mg tablet still follows the eight-hour average in a healthy adult. The labelled alcohol hold keeps going for at least three days after that swallow. Half-life is not permission to drink at hour nine.
Breast milk can carry metronidazole at concentrations similar to maternal serum. A nursing mother may pump and discard during therapy and for 48 hours after the last dose. That is a milk clock, not the eight-hour plasma number.
03Why eight hours is not the drink clock
Ethanol and propylene glycol stay banned on the insert during therapy and for at least three days after the last dose. That hold is written in days, not in half-lives. Count it on the three-day probe.
The glow list (cramps, nausea, vomiting, headache, flushing) lives on the alcohol pulse. The eight-hour curve does not retell that scene.
04What eight hours describes
Average elimination half-life in healthy subjects is eight hours. Parent drug and the hydroxy metabolite both show antimicrobial activity in vitro. Unchanged metronidazole is about 20 percent of recovered material in urine.
Feces account for 6 to 15 percent of the dose. Less than 20 percent of circulating metronidazole is bound to plasma proteins. Those are PK rows, not a reason to redose early.
05When the eight-hour average moves
After 500 mg IV, mean AUC24 rose 114 percent in Child-Pugh C hepatic impairment versus healthy controls, and about 54 and 53 percent in Child-Pugh A and B. Severe impairment gets a 50 percent dose cut.
Mild to moderate hepatic impairment needs monitoring, not that automatic halving. Geriatric patients can show higher hydroxy-metabolite AUC. Newborn half-lives in the first three days of life ran from 109 to 22.5 hours across 28 to 40 weeks' gestation.
06Where the 250 mg peak sits
A 250 mg oral dose produced peak plasma near 6 mcg/mL in the dose-proportional set. 500 mg sat near 12 mcg/mL. 2,000 mg sat near 40 mcg/mL. Peaks occur between one and two hours.
Males and females did not show a bioavailability split in the studies the insert cites. Weight differences still leave men with generally lower plasma levels.
